What to Bring to Your First Visit
Your child's insurance card, a photo ID for the parent or guardian, completed new patient paperwork, any previous medical records or immunization history, and a list of current medications.
Getting Started Is Easy
Call us at 530-487-7111 during regular business hours, Monday through Friday, 8:00 AM to 5:00 PM. We'll verify your insurance, schedule your first appointment, and send over new patient paperwork to complete before your visit. We can't wait to meet your family.
